{"id":46179,"date":"2026-04-07T13:14:47","date_gmt":"2026-04-07T17:14:47","guid":{"rendered":"https:\/\/rehab-u.com\/?p=46179"},"modified":"2026-04-07T13:21:49","modified_gmt":"2026-04-07T17:21:49","slug":"rehab-du-lca-entrainement-du-controle-dynamique-du-valgus","status":"publish","type":"post","link":"https:\/\/rehab-u.com\/fr\/rehab-du-lca-entrainement-du-controle-dynamique-du-valgus\/","title":{"rendered":"Rehab du LCA : Entra\u00eenement du contr\u00f4le dynamique du valgus"},"content":{"rendered":"<p>Le valgus dynamique du genou est fr\u00e9quemment discut\u00e9 en rehab du LCA et dans l\u2019entra\u00eenement du retour au sport, particuli\u00e8rement lors des changements de direction, des atterrissages et des d\u00e9c\u00e9l\u00e9rations.<\/p>\n<p>Parce qu\u2019il a \u00e9t\u00e9 associ\u00e9 au risque de blessure du LCA, de nombreuses strat\u00e9gies de rehab tentent d\u2019\u00e9liminer compl\u00e8tement le valgus du genou, en le d\u00e9crivant comme une \u00ab position \u00e0 haut risque \u00bb. Les athl\u00e8tes sont souvent entra\u00een\u00e9s \u00e0 garder les genoux vers l\u2019ext\u00e9rieur, \u00e0 maintenir un alignement parfait et \u00e0 \u00e9viter d\u2019entrer dans ces positions pendant le mouvement.<\/p>\n<p>Mais lorsque l\u2019on observe comment les athl\u00e8tes bougent r\u00e9ellement en sport, ces positions ne sont pas seulement fr\u00e9quentes \u2014 elles sont n\u00e9cessaires pour g\u00e9rer les forces de r\u00e9action au sol lors des changements de direction.<\/p>\n<p>La v\u00e9ritable question n\u2019est donc pas comment \u00e9liminer le valgus dynamique du genou, mais plut\u00f4t :<\/p>\n<p><strong>Comment pr\u00e9parer les athl\u00e8tes \u00e0 le g\u00e9rer ?<\/strong><\/p>\n<p><iframe loading=\"lazy\" title=\"ACL Rehab: Training Dynamic Knee Valgus Control\" width=\"1080\" height=\"608\" src=\"https:\/\/www.youtube.com\/embed\/AchjMVsQJwQ?feature=oembed&#038;enablejsapi=1&#038;origin=https:\/\/rehab-u.com\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe><\/p>\n<p>REGARDEZ LA VID\u00c9O COMPL\u00c8TE POUR OBTENIR TOUS LES D\u00c9TAILS (CLIQUER SUR LA ROUE DENTEL\u00c9E EN HAUT \u00c0 DROITE POUR ACTIVER LES SOUS-TITRES EN FRAN\u00c7AIS)<\/p>\n<h2 data-section-id=\"qrob7h\" data-start=\"1249\" data-end=\"1300\">Le probl\u00e8me de nombreux programmes de rehab du LCA<\/h2>\n<p>L\u2019une des principales limitations de nombreux programmes de rehab du LCA est le manque de structure et de progression claires.<\/p>\n<p>Les strat\u00e9gies courantes incluent souvent :<\/p>\n<ul>\n<li>\u00e9viter compl\u00e8tement le valgus du genou<\/li>\n<li>coacher constamment la position \u00ab genoux vers l\u2019ext\u00e9rieur \u00bb<\/li>\n<li>s\u2019appuyer sur des strat\u00e9gies anti-rotation<\/li>\n<li>utiliser des consignes tr\u00e8s centr\u00e9es sur l\u2019attention interne<\/li>\n<\/ul>\n<p>Par exemple, on utilise souvent des consignes comme :<\/p>\n<ul>\n<li>\u00ab Stabilise ton atterrissage. \u00bb<\/li>\n<li>\u00ab Garde ton genou vers l\u2019ext\u00e9rieur. \u00bb<\/li>\n<li>\u00ab Concentre-toi sur ton alignement. \u00bb<\/li>\n<\/ul>\n<p>Ces consignes mettent l\u2019accent sur la position consciente des articulations, ce qui ne refl\u00e8te pas les exigences des environnements sportifs r\u00e9els.<\/p>\n<p>En sport, les athl\u00e8tes ne pensent pas \u00e0 la position exacte de leur genou lorsqu\u2019ils coupent, d\u00e9c\u00e9l\u00e8rent ou atterrissent. Ils r\u00e9agissent plut\u00f4t \u00e0 des contraintes externes telles que :<\/p>\n<ul>\n<li>le ballon<\/li>\n<li>un adversaire<\/li>\n<li>la direction du mouvement<\/li>\n<li>le timing de l\u2019action<\/li>\n<\/ul>\n<p>Une autre limite fr\u00e9quente est l\u2019\u00e9cart entre les exercices de renforcement de base et les exercices plyom\u00e9triques avanc\u00e9s.<\/p>\n<p>De nombreux programmes de rehab du LCA passent directement du renforcement traditionnel aux exercices de saut et d\u2019atterrissage, sans exposer d\u2019abord les athl\u00e8tes \u00e0 des positions rotationnelles sous charge dans des environnements plus lents et contr\u00f4l\u00e9s.<\/p>\n<p>Cette \u00e9tape est pourtant essentielle pour d\u00e9velopper la force, le contr\u00f4le et la tol\u00e9rance tissulaire n\u00e9cessaires pour g\u00e9rer les forces rencontr\u00e9es en sport.<\/p>\n<h2 data-section-id=\"9j488f\" data-start=\"2592\" data-end=\"2642\">Le valgus dynamique du genou est un sch\u00e9ma de mouvement normal<\/h2>\n<p>Le cadre propos\u00e9 par <a href=\"https:\/\/doi.org\/10.2519\/jospt.2019.8788\" target=\"_blank\" rel=\"noopener\">Dischiavi et al. (2019)<\/a> encourage les cliniciens \u00e0 repenser la mani\u00e8re dont le valgus dynamique du genou est interpr\u00e9t\u00e9 en rehab du LCA.<\/p>\n<p>Plut\u00f4t que de le consid\u00e9rer uniquement comme une position pathologique, les auteurs le d\u00e9crivent comme un mouvement triplanaire normal utilis\u00e9 pour g\u00e9rer les forces de r\u00e9action au sol.<\/p>\n<p>Ce mouvement implique une coordination entre plusieurs segments du corps, notamment :<\/p>\n<ul>\n<li>le tronc<\/li>\n<li>le bassin<\/li>\n<li>le f\u00e9mur<\/li>\n<li>le genou<\/li>\n<li>la cheville et le pied<\/li>\n<\/ul>\n<p>Cette coordination tridimensionnelle permet au corps d\u2019absorber et de redistribuer les forces lors de mouvements athl\u00e9tiques comme les changements de direction, les atterrissages et les d\u00e9c\u00e9l\u00e9rations.<\/p>\n<p>Pour cette raison, le valgus dynamique du genou ne peut pas \u00eatre \u00e9limin\u00e9 de mani\u00e8re r\u00e9aliste en sport.<\/p>\n<p>Les athl\u00e8tes doivent plut\u00f4t d\u00e9velopper la capacit\u00e9 de g\u00e9rer ces positions sous des charges, des vitesses et des niveaux d\u2019impr\u00e9visibilit\u00e9 croissants.<\/p>\n<h2 data-section-id=\"wdifq3\" data-start=\"3474\" data-end=\"3520\">Appliquer ce cadre \u00e0 la rehab du LCA<\/h2>\n<p>Selon cette perspective, la programmation de la r\u00e9\u00e9ducation du LCA devrait pr\u00e9parer progressivement les athl\u00e8tes \u00e0 travers trois niveaux :<\/p>\n<ul>\n<li>Mobilit\u00e9 et contr\u00f4le<\/li>\n<li>Force et absorption des forces<\/li>\n<li>Exigences athl\u00e9tiques<\/li>\n<\/ul>\n<h3 data-section-id=\"rqwpvf\" data-start=\"3640\" data-end=\"3664\">1\ufe0f\u20e3 Mobilit\u00e9 et contr\u00f4le<\/h3>\n<p>En rehab, notre r\u00f4le est de donner aux athl\u00e8tes les ressources n\u00e9cessaires pour bouger de mani\u00e8re s\u00e9curitaire et efficace. Les athl\u00e8tes doivent pouvoir acc\u00e9der et contr\u00f4ler les positions n\u00e9cessaires pour absorber les charges lors des t\u00e2ches unipodales.<\/p>\n<p>Deux \u00e9l\u00e9ments cl\u00e9s contribuent \u00e0 cela :<\/p>\n<p><strong>Rotation interne de la hanche<\/strong><\/p>\n<p>Une rotation interne ad\u00e9quate de la hanche permet au membre inf\u00e9rieur de participer efficacement \u00e0 l\u2019absorption des charges et \u00e0 la distribution des forces lors des changements de direction.<\/p>\n<p>Les recherches ont montr\u00e9 qu\u2019une plus grande rotation interne de la hanche est associ\u00e9e \u00e0 de meilleures performances et \u00e0 des charges plus favorables au niveau du genou lors des changements de direction \u00e0 angles plus prononc\u00e9s.<\/p>\n<p>Il est important de noter que la rotation de la hanche ne doit pas \u00eatre \u00e9valu\u00e9e isol\u00e9ment. La rotation tibiale et la m\u00e9canique du complexe cheville-pied (pronation et supination) doivent \u00e9galement \u00eatre \u00e9valu\u00e9es, car ces facteurs contribuent aux m\u00e9caniques rotationnelles de l\u2019ensemble de la cha\u00eene cin\u00e9tique.<\/p>\n<p><strong>Dissociation bassin\u2013f\u00e9mur<\/strong><\/p>\n<p>Un autre \u00e9l\u00e9ment cl\u00e9 est la capacit\u00e9 \u00e0 d\u00e9placer le bassin au-dessus du f\u00e9mur tout en maintenant le contr\u00f4le du tronc et de la hanche.<\/p>\n<p>Cette relation permet aux athl\u00e8tes d\u2019acc\u00e9der aux m\u00e9caniques rotationnelles n\u00e9cessaires pour absorber les forces lors des charges sur une seule jambe.<\/p>\n<p>Comme l\u2019ont soulign\u00e9 Dischiavi et ses coll\u00e8gues :<\/p>\n<p style=\"text-align: center;\">\u00ab Un mouvement socket-over-ball se produisant dans trois plans est n\u00e9cessaire pour faciliter l\u2019absorption des forces de r\u00e9action au sol et constitue une composante essentielle du mouvement humain. \u00bb<\/p>\n<h3 data-section-id=\"fe73qf\" data-start=\"4961\" data-end=\"4994\">2\ufe0f\u20e3 Force et capacit\u00e9 d\u2019absorption des forces<\/h3>\n<p>Un deuxi\u00e8me \u00e9l\u00e9ment crucial dans la programmation de la rehab du LCA est le d\u00e9veloppement de la force sp\u00e9cifique n\u00e9cessaire pour g\u00e9rer les mouvements triplanaires lors des charges unipodales.<\/p>\n<p>Deux \u00e9l\u00e9ments doivent \u00eatre particuli\u00e8rement d\u00e9velopp\u00e9s :<\/p>\n<p><strong>Force rotationnelle sous charge<\/strong><\/p>\n<p>Les exercices doivent \u00eatre choisis en fonction de leur capacit\u00e9 \u00e0 reproduire les orientations articulaires et les m\u00e9caniques rotationnelles des charges unipodales, mais sous une charge externe plus importante.<\/p>\n<p>Ces exercices impliquent g\u00e9n\u00e9ralement le d\u00e9placement du tronc et du bassin au-dessus d\u2019un f\u00e9mur fixe, ce qui aide les athl\u00e8tes \u00e0 d\u00e9velopper une tol\u00e9rance aux forces rotationnelles dans des environnements contr\u00f4l\u00e9s.<\/p>\n<p><strong>Capacit\u00e9 d\u2019absorption des forces<\/strong><\/p>\n<p>Les exercices de renforcement doivent progressivement exposer les athl\u00e8tes \u00e0 des charges plus \u00e9lev\u00e9es et \u00e0 des vitesses de contraction plus rapides, servant de pont entre l\u2019entra\u00eenement de force traditionnel et les exigences dynamiques du retour au sport.<\/p>\n<p><strong>ASSUREZ-VOUS DE REGARDER LA VID\u00c9O COMPL\u00c8TE POUR VOIR DES athl\u00e9tiques<\/h3>\n<p data-start=\"5943\" data-end=\"6061\">Enfin, la programmation de la rehab du LCA doit pr\u00e9parer les athl\u00e8tes \u00e0 g\u00e9rer ces m\u00e9caniques rotationnelles dans les v\u00e9ritables exigences du sport, o\u00f9 les mouvements se produisent sous vitesse, impr\u00e9visibilit\u00e9 et contraintes de t\u00e2che.<\/p>\n<p data-section-id=\"ifi2a6\" data-start=\"6160\" data-end=\"6197\"><strong>Comprendre l\u2019instabilit\u00e9 en sport<\/strong><\/p>\n<p>L\u2019instabilit\u00e9 est souvent mal comprise en rehab.<\/p>\n<p>De nombreux programmes tentent de cr\u00e9er de l\u2019instabilit\u00e9 en utilisant des surfaces instables, comme des BOSU ou des planches d\u2019\u00e9quilibre.<\/p>\n<p>L\u2019entra\u00eenement sur ces surfaces provoque principalement une r\u00e9ponse au basculement de la surface, o\u00f9 l\u2019athl\u00e8te r\u00e9agit au d\u00e9placement angulaire de la base de support.<\/p>\n<p>Ce type d\u2019entra\u00eenement peut \u00eatre utile au d\u00e9but de la rehab, notamment apr\u00e8s une blessure articulaire lorsque la proprioception et l\u2019\u00e9quilibre sont perturb\u00e9s.<\/p>\n<p>Cependant, cela ne refl\u00e8te pas la fa\u00e7on dont l\u2019instabilit\u00e9 se produit en sport.<\/p>\n<p>En sport, la surface est g\u00e9n\u00e9ralement stable et l\u2019instabilit\u00e9 survient lorsque le centre de masse est perturb\u00e9 alors que l\u2019athl\u00e8te reste sur une surface stable.<\/p>\n<p data-section-id=\"1ok2efs\" data-start=\"6910\" data-end=\"6943\"><strong>Contr\u00f4le neuromusculaire r\u00e9actif<\/strong><\/p>\n<p>Pour reproduire ces exigences, l\u2019entra\u00eenement doit introduire des perturbations impr\u00e9vues qui obligent les athl\u00e8tes \u00e0 retrouver rapidement leur \u00e9quilibre.<\/p>\n<p>Par exemple :<\/p>\n<ul>\n<li>r\u00e9agir \u00e0 un objet en mouvement<\/li>\n<li>r\u00e9pondre \u00e0 des signaux directionnels<\/li>\n<li>absorber des forces inattendues<\/li>\n<\/ul>\n<p>Ces exercices refl\u00e8tent mieux la nature r\u00e9active du sport, o\u00f9 les athl\u00e8tes doivent constamment s\u2019adapter aux contraintes externes.<\/p>\n<p><strong>ASSUREZ-VOUS DE REGARDER LA VID\u00c9O COMPL\u00c8TE POUR VOIR \u00c0 QUOI CELA RESSEMBLE EN PRATIQUE.<\/strong><\/p>\n<h2 data-section-id=\"z0xfk0\" data-start=\"7324\" data-end=\"7371\">Un changement de perspective pour la programmation en rehab du LCA<\/h2>\n<p>Le valgus dynamique du genou ne devrait pas \u00eatre consid\u00e9r\u00e9 uniquement comme une position \u00e0 \u00e9liminer.<\/p>\n<p>Au contraire, les <a href=\"https:\/\/rehab-u.com\/fr\/programme-de-prevention-base-sur-lexercice-pour-les-blessures-du-lca\/\" target=\"_blank\" rel=\"noopener\">programmes de rehab du LCA<\/a> devraient pr\u00e9parer progressivement les athl\u00e8tes \u00e0 acc\u00e9der \u00e0 ces positions, les contr\u00f4ler et les tol\u00e9rer, sous des charges et des niveaux d\u2019impr\u00e9visibilit\u00e9 croissants.<\/p>\n<p>En structurant la r\u00e9\u00e9ducation autour de trois \u00e9tapes :<\/p>\n<p>Mobilit\u00e9 et contr\u00f4le<br \/>\nForce et absorption des forces<br \/>\nExigences athl\u00e9tiques<\/p>\n<p>les professionnels de la r\u00e9\u00e9ducation peuvent mieux pr\u00e9parer les athl\u00e8tes \u00e0 g\u00e9rer les forces complexes rencontr\u00e9es lors des changements de direction, des atterrissages et du retour au sport.<\/p>\n<p>Au final, l\u2019objectif n\u2019est pas d\u2019\u00e9liminer la variabilit\u00e9 du mouvement,
